Patients are seen by appointment only. To schedule an appointment call our main office at (608) 257-2208. Our office hours are 8AM to 4PM. Monday through Friday. We attempt to schedule our appointments at intervals to maintain a relaxed atmosphere and to minimize waiting. If you anticipate being late for your appointment, please call as soon as possible and we will be happy to accommodate you!
Emergencies
In the event that you require emergency care or have urgent problems, staff can be reached 24 hours a day by calling our office (608)257-2208. During off hours, an answering service will provide you with the system to contact the particular staff member on call. One of our staff is available for you 24 hours each day.
Insurance
We are not affiliated with any of the major HMO's in the area. We do work
with several independant insurance company's.
Payment
We do require that payment is made in full 3 weeks prior to the date of a
patients surgical procedure. We accept VISA, Master Card, American Express, & Discover. We also accept personal checks, cashiers checks and cash.
